Emergency Form

Food Allergy Action Plan

Food allergies require an immediate response, and can be life threatening. If you have a child with a severe food allergy, be sure your babysitter is prepared to respond appropriately by leaving her all necessary information, instructions and medication, including prescribed, self-injectable Epinephrine.
